The article describes the modified surgical approach to the atherosclerotic lesion of the carotid and subclavian arteries. The main advantage of this approach is the possibility of the connection of both carotid and subclavian arteries through the single incision. The immediate and long-term results have been evaluated. There was no death or myocardial infarction during the early postoperative period. One (3.3%) stroke was registrated in 30 days after surgery. One death of the patient with generalized atherosclerosis was registered in the long-term period (the average follow up – 26.1 months) Total patency is 90%.
