Biochemical parameters and morphofunctional state of the liver in evaluating the prediction of disease in patients with mechanical jaundice, liver insufficiency and principles for corrective therapy

Hepatic insufficiency is a complex of clinical symptoms that arise as a result of the violation of compensatory possibilities and functions of the organ, as a result of which the liver can not maintain homeostasis in the body and provide it with a normal metabolism. There are a lot of reasons for the development of liver failure, but regardless of them in liver cells (hepatocytes) there are always the same changes. Hepatocytes are extremely sensitive to lack of oxygen, so in certain conditions, liver failure can develop very quickly and lead to death. There are a large number of prognostic tests of outcomes of acute liver failure in the world, but they are not always able to predict the course of the disease and have limited value. Mortality among patients with this pathology remains high despite modern methods of treatment and reaches 10.5-42.9%. The article provides information on the determination of the severity of liver failure due to mechanical jaundice, based on the evaluation of the functional morphology of the liver and histopathological changes in the hepatic parenchyma. Recommendations for management of patients and surgical tactics are given depending on the severity of acute liver failure based on the morphofunctional method.
