in this article, based on the analysis of legal material and legal literature, legal forms of obtaining consent are considered, the forms of treatment are presented, and the author comes to the conclusion that non-compliance with the form of application for obtaining consent, lack of necessary documents and materials in electronic form, non-compliance with conditions, violation of the form of treatment entails negative consequences for the person, the subject of legal relationship.
