Objective. Detection of pathological polymorphisms of endothelial synthase, causing significant damage in the gene structure in patients with resistant hypertension (RAH). Materials and methods. In our study, the main group consisted of 98 people diagnosed with resistant arterial hypertension (RAH) (mean age 60.4±7.2 years), who underwent a study to detect polymorphism of the endothelial nitric oxide synthase (NO3) gene. The control group consisted of patients diagnosed with essential arterial hypertension (EG) - 62 persons (mean age 55.3±7.2 years). All patients were comparable by sex and age. In patients, genotype frequencies in the study groups were determined by PCR (polymerase chain reaction) with real-time detection of results; analysis of melting curves, qualitative analysis. Material for the study: peripheral blood. Results. The group of patients with RAH and the group of patients with EG (essential hypertension) differed in the frequency of occurrence of all genotypes. There were statistically significant differences in the incidence of heterozygous and homozygous genotypes (p<0.05), when comparing groups of patients with RAH and EG Findings. Conclusions. The study proved that the occurrence of RAH is caused by polymorphisms of different genes, it undoubtedly requires further study of genetic factors affecting the development of RAH.
