Numerous problems associated with impaired reproductive function in women have shown that gender and sex steroids affect the composition of the intestinal and vaginal microbiome. Aim of this review is to summarize the available literature data about the relationship between microbiome and PCOS. Most of evidence has focused on the connection of intestinal bacteria with sex hormones and insulin-resistance. Studies show that steroid regulation of the gut microbiome may play a role in pathological situations of hormonal overload such as PCOS, in particular androgens may regulate the gut microbiome in women, and that hyperandrogenism may be associated with gut dysbiosis in PCOS. Understanding this fine‐tuned interaction is key to maintaining women's reproductive health.

Keywords:polycystic ovary syndrome, hyperandrogenism, insulin resistance, gut microbiome, vaginal microbiome.
