Forefoot deformities are common in patients with rheumatoid arthritis and are often debilitating. Chronic inflammation leads to the formation of pannus, weakening of the ligaments and destruction of the joints, which ultimately leads to multiple deformities of the joints and changes in the biomechanics of the foot. Claw-like and hammer-like deformities of the fingers, as well as hallux valgus of the 1st toe, are the most common and require surgical treatment if conservative treatment is ineffective. This review discusses the pathophysiology, biomechanical effects, and available treatment options for rheumatoid deformities of the forefoot. It is imperative that clinicians, especially those who specialize in the treatment of foot and ankle pathology, have a good understanding of these concepts to optimize patient outcomes.
