CLINICAL CHARACTERISTICS OF INFECTIOUS ENDOCARDITIS IN THE EXAMPLE OF PATIENTS WITH RISK FACTORS

The purpose of the analysis of these clinical cases was to study the main features of the clinical and laboratory-instrumental picture, the principles of antibiotic therapy, as well as complications of infective endocarditis in patients with risk factors. Two clinical cases of infective endocarditis in patients with risk factors who were treated in the cardiology department of the Republican Hospital No. 1 - the National Center of Medicine of the city of Yakutsk are presented. It has been practically shown that a significant role in the diagnosis of infective endocarditis is played by the correct, careful questioning of anamnestic data. In practice, the low role of the Duke criteria in the diagnosis of infective endocarditis is noticeable, a large role is given to the visualization of vegetations by echocardiography, and microbiological diagnosis of blood cultures for sterility is also important. In the treatment of patients, multiple schemes of combined antibiotic therapy were used, taking into account the data of blood cultures. During the treatment period, the patients showed normalization of laboratory parameters with the preservation or progression of vegetations, the clinical picture of the current infectious process, which may be associated with an atypical course of infective endocarditis in patients with risk factors. Both patients had absolute indications for surgery in the form of uncontrolled infection, large vegetations, development of severe tricuspid valve regurgitation, concomitant conditions, complications such as heart failure, and sepsis.
