CANCELLATION AND CORRECTION OF ANTICOAGULANT AND ANTIPLATELET THERAPY BEFORE SURGICAL DENTAL INTERVENTIONS

In the clinical practice of a dentist, a patient with circulatory system diseases and rhythm disorders usually receives anticoagulants, which reduces the risk of cardiovascular disasters such as myocardial infarction and stroke, but at the same time increases the risk of bleeding during dental procedures. Various studies and meta-analyses show that taking anticoagulants and antiplatelet agents increases the likelihood of bleeding during surgical operations. Therefore, this topic is relevant for research and requires further consideration. The aim of the study was to determine the optimal management tactics for patients receiving anticoagulant and antiplatelet therapy in the preparation and conduct of surgical dental interventions, taking into account the risk of bleeding and thromboembolic complications. Materials and methods. The research material was scientific articles from leading specialized journals and textbooks on this topic. The data was searched and analyzed in the databases of Mendeley, Web of Science, EMBASE and the Russian National Library. The research methods included a systematic review of the literature, analysis of the data obtained and synthesis of the results. The results of the study. Most patients with cardiac arrhythmias receive anticoagulant therapy, improving the condition of the endothelium and microcirculation in the oral cavity. However, dental procedures when taking anticoagulants can cause complications in the hemostasis system, and their cancellation can cause thromboembolic phenomena. There are many methods to solve this problem, but the optimal method has not been determined. The conducted literature review showed that there are unresolved issues that require further study and research. Therefore, today the treatment of patients with cardiac arrhythmias while taking anticoagulants is a difficult and urgent task of modern medicine. Conclusions. Optimal treatment tactics for patients taking anticoagulants and antiplatelet agents during dental procedures requires an individual approach and coordination of actions by dentists and cardiologists. Further research is needed to develop clearer protocols and standards of care to minimize the risk of bleeding and thromboembolism.
