the aim of this study was to determine the impact of anesthetic tactics, in particular the use of navigation-controlled TAP block with ropivacaine, and comorbid pathology on the course of the early postoperative period in patients who underwent surgery for colorectal cancer. The prospective study involved 106 patients divided into groups with and without TAP block. The severity of pain, the need for analgesics, the restoration of gastrointestinal motility and the duration of hospitalization were assessed depending on the Charlson comorbidity index and the presence of specific comorbidities. The results showed that TAP block helps reduce pain, decrease the opioid load, accelerate functional recovery and potentially reduce the duration of hospitalization. The most pronounced positive effect was noted in patients with high somatic burden. These data support the advisability of including TAP block in ERAS protocols in comorbid patients with colorectal cancer.

Keywords:colorectal cancer; TAP block; regional anesthesia; postoperative period; comorbidity.
