DIAGNOSIS AND INTERVENTIONAL TREATMENT OF CAROTID PATHOLOGY. A BRIEF HISTORICAL EXCURSION. FEATURES OF INTERVENTIONAL BENEFITS FOR COMBINED CAROTID AND CARDIAC PATHOLOGIES

The aim of this study was to evaluate the safety and efficacy of internal carotid artery (ICA) stenting in patients with severe concomitant cardiac pathology. The study included data from 250 patients who underwent internal carotid artery stenting against the background of comorbid cardiovascular disease.Keywords: carotid stenosis, carotid endarterectomy, carotid artery stenting, combined carotid and cardiac pathology, hybrid treatment strategies. Dye to the type of the therapeutic approach, patients were divided into two groups: Group 1, the 'study group' – patients who underwent staged treatment of carotid and cardiac pathology (n=129, 51.6%); Group 2, the 'comparison group' – patients who underwent ICA stenting with the background of compensated cardiac pathology in their anamnesis (n=121, 48.4%). The results of this study showed that in the long-term period, the staged treatment group for carotid and cardiac pathology had significantly fewer cases of ischemic stroke (p=0.04 at 12 months; p=0.0006 at 36 months), however, the number of ischemic coronary events was significantly higher (p=0.003 at 12 months; p=0.03 at 36 months), which could be related to the initial severity of the patients. The presented strategies for staged endovascular treatment of ICA lesions and correction of cardiac pathology are effective and safe, and do not differ from the standard outcomes of isolated ICA stenting.
