INTERACTIVE QUESTIONNAIRE AND DIGITAL CLINICAL DECISION SUPPORT SYSTEM IN PRELIMINARY DIFFERENTIAL DIAGNOSIS OF DISEASES OF THE UPPER GASTROINTESTINAL TRACT

The aim of the study is to determine the diagnostic and prognostic significance of individual signs detected by an automated interactive survey system in the preliminary differential diagnosis of diseases of the upper gastrointestinal tract. The analysis of interactive questionnaire data using the gastroenterological module of the automated medical survey system was carried out in 192 patients with verified diseases of the upper gastrointestinal tract: 47 with chronic gastritis and/or duodenitis, 43 with peptic ulcer of the stomach or duodenum, 35 with chronic pancreatitis, 34 with chronic cholecystitis and 33 with with stage I-III gastric cancer. The logistic regression method was used to build predictive models for binary dependent variables. According to the results of an interactive survey, patients with stomach cancer were statistically significantly more likely to experience pain of combined localization and a high prevalence of non-gastroenterological symptoms such as fever and weight loss; and gastric dyspepsia syndrome was significantly more common in non-cancerous diseases of the upper gastrointestinal tract than in stomach cancer. Regression models of diseases of the upper gastrointestinal tract, obtained based on the results of an automated interactive survey system, have demonstrated high diagnostic effectiveness and can be used for a preliminary differentiated approach to choosing an individual plan for further examination of the patient at the stage of additional laboratory and instrumental studies.
