The formation of the infrastructure of medical institutions and the medical community in Samara was studied in article in the context of its transformation into a gubernia center. The methodology of local history became the basis of the research. The use of historical-genetic and problem-chronological methods helped to identify the free stages in the development of the medical care system in the pre-reform provincial city: 1) the end of the XVIII – the first quarter of the XIX century – the formation of urban medicine, the appearance of the first medical staff in the province. 2) the end of the 20s – 40s of the XIX century – the appearance of the first medical institutions in Samara and the formation of the medical community; 3) the 50s – early 60s of the XIX century – the formation of a medical care system for the urban population, the development of organizational and infrastructural forms of urban medicine in the context of a large-scale influx of population to Samara in connection with its transformation into the provincial capital.
